Immunization information is now being accepted online through the Waterloo Region Public Health website at: https://e-immunization.regionofwaterloo.ca/. This website link is available for parents on your school website on the ‘Register for Kindergarten’ page.

For parents unable to submit the information online, the Form E must be completed in paper form and sent to Public Health.
